There’s no one-size-fits-all approach to aged care. All our residents are treated as individuals, which means their services and support are designed to suit them. We offer comprehensive nursing care for those living with dementia, older people in need of support, or for those looking for respite or convalescent care.

‍Our multi-disciplinary care team includes doctors, nurse prescribers, nurses, physiotherapists, occupational therapists, dietitians, social workers and members of the spiritual and religious care community.

Long term care

If someone you love has a medical condition that requires 24-hour care that can’t be given at home, a nursing home may be the right choice. Our nurses are trained to care for people with complex medical conditions and to recognise the signs and symptoms of illness, providing family members and residents the comfort of knowing their health is being managed appropriately.

We understand that the decision to move from the family home can be an emotional one. That’s why we take the time to really get to know our residents, understanding what they like and don’t like, and what they enjoy doing. This way, we can offer a truly seamless continuity of care that helps residents settle in comfortably and easily to their new life.

Respite care

When you’re caring for someone, it’s important to care for yourself, too. Grace Healthcare offers nurturing, professional respite care in the home, giving the primary carer the chance to take a break. It’s a vital service that lets you relax, knowing your relative, friend or patient is in safe hands while you’re away.

Convalescence care

We welcome residents who need our services to rest and recover for a short time, usually a week or two, after leaving hospital. Our highly skilled and compassionate staff make sure that residents are ready to return home safely, and feeling well.  

Dementia care

We want all our residents to live fulfilling lives with us. Living with dementia requires sensitive, informed caring that treats residents as individuals. We take the time to learn how our residents with dementia wish to live their life and what kind of help they will need. Our well-appointed homes are designed to promote independence within a sanctuary of support.

We work closely with dementia care experts so that everything we do is informed by the latest thinking and best practice. By creating the right environment and tailoring our care, we can help minimise anxiety and encourage the wellbeing and comfort of all our residents.

Many residents have lived at home before joining us, often cared for by a family member or a friend. We know how hard that can be for carers and part of our process is helping relatives understand more about the condition of dementia.
